Please print, sign, and mail your application with a $60 non-refundable application fee, payable to “Gates and Bridges Preschool”  to our attention at 1601 10th Avenue, San Francisco, CA 94122. Your application will be retained until your child is no longer age eligible, you decline an enrollment offer, or you request to be removed from the waiting list.
